This form is used to report testing issues related to the IPAWS-WEA and B-WARN! system during regularly scheduled tests on the First Friday of the month between 3PM and 4PM and for any real incident alerts. It may also be used to request help with B-WARN! accounts.

2. I want to change my B-WARN! settings

1. Live Alert for Real Incident?
2. Test?
I did NOT receive an alert for:
1. IPAWS - Wireless Emergency Alerts
2. B-WARN - Sign Up Service

3. I heard someone else's alert or became aware another way
4. NOAA Weather Radio (Live Incident Only)
5. Marine Band Radio (Live Incident Only)
